ABSENCE EXCUSE FORM

If you plan to be absent from a future rehearsal or performance, please fill out the following form and email Mr. Ramos at ramos.david@mybradford.us with the date and reason for absence.

If you have not yet done so, please fill out the commitment form to show your intent to take part in the Bradford High School Band Program for the 2022 - 23 season, and email it to ramos.david@mybradford.us.

EXEMPTIONS FORMS

Participation in the Scarlet Storm Marching Band is required of all band students participating in one of our concert bands or our jazz band. If you have a medical concern or fall sport conflict with participating in the marching band, please fill out either the Fall Sport Exemption Form or the Medical Exemption Form, and email it to ramos.david@mybradford.us.

INSTRUMENT RENTAL FORM

The Bradford High School Band offers a wonderful opportunity to rent school owned instruments to band students at an extremely reduced rate - $25 a semester! If you would like to rent a school owned instrument, please fill out the Instrument Rental Form and email it to ramos.david@mybradford.us. Cash and checks made out to "Bradford High School Band" will be accepted at either the school office or band office.

MEDICAL HISTORY FORM

The Medical History Band Camp form is to be filled out at the start of band camp to help the director keep track of important medical concerns. Students and guardians should know information provided in this form is sensitive. Only the director will have access to this at any time during the school year.

OFF CAMPUS FORM

The Bradford County School District Off-Campus School Activity Consent Form is a district mandated form required of all students to permit their participation in away game, assessment, and program sponsored trip travel.

VOLUNTEER FORM

The Bradford County Public Schools Volunteer/Chaperone/School Activities Application (Volunteer Form) is a district mandated form for any adult interested in volunteering for the Bradford Band Program. Volunteering for the program includes but is not limited to chaperoning events and travel, and working weekly concession stands.
